1. Duly accomplishd AFPEBSO Educational Benefit Application Form attached with two (2) copies of llatest 2 X 2 pictures of the applicant.

2. One (1) copy or authenticated machine copy of birth certificate of the applicant

3. One (1) copy or authenticated machine copy of marriage contract of the military personnel.

4. One (1) copy or authenticated machine copy of report card (Form 138), Certification of Grades or Transcript of Records (TOR) of the applicant from the most recent educational level accomplished and educational institution attended.

5. Certificate of Good Moral Character of the applicant from the last educational institution attended.

6. If the applicant is a dependent of KIA military personnel, he/she must submit a copy or authenticated copy of his/her parent's death certificate, casualty and posthumous report.

7. If the applicant is an active military person , he/she must submit the following: clearances from the TJAG, TIG, and TPMG offices; one (1) authenticated copy of the letter of recommendation from the concerned Commanding Officer; one (1) authenticated copy of Reenlistment Order for enlisted personnel; and Summary of Information [SOI] and Statement of Service.
